Hmm, looks like the sbxvi driver has a dependancy on the sbmidi driver (dumb or what..?) you should add that too :) A line like : 'device sbmidi0 at isa? port 0x388' should do it.. I get sbmidi0 not found at 0x388 though.. Perhaps you could try 0x380 instead..
