It works with both PAM and sasldb for me. There is some issues with sasl 1.5.15 and realms that may be what you have seen. I think it was fixed in 1.5.18, at least partly. For more info see: http://www.sendmail.org/~ca/email/authrealms.html. Also check that sasldb is owned by the cyrus user and that it's mode 600. Otherwise sasl won't use it.
