Just for "shits and giggles" I tested this both with sshd all upper case and all lower case and both worked. Make an /etc/hosts.allow with one single line: sshd : ALL : deny and work it from there, as others have suggested, there may be other lines that are superceeding this one. Also, it's been my experience that things work better when the colons have spaces on each side of them. (as opposed to sshd:all:deny).
