Hi,

It appears your USB Host Controller Hardware has crashed.

Could you boot having the following line in /boot/loader.conf:

hw.usb2.uhci.debug=15

And send new dmesg.



Patch you can try in the meanwhile:

Edit /sys/dev/usb/controller/usb_controller.c:

-	usb2_dma_tag_setup(bus->dma_parent_tag, bus->dma_tags,
-	    dmat, &bus->bus_mtx, NULL, 32, USB_BUS_DMA_TAG_MAX);

+	usb2_dma_tag_setup(bus->dma_parent_tag, bus->dma_tags,
+	    dmat, &bus->bus_mtx, NULL, 30, USB_BUS_DMA_TAG_MAX);

This will reduce the DMA bits to 30.
