I once discussed this problem with Simon, and it turned out that the version on that card was newer. Reverting to an older version fixed the problem.

I'm currently running:
dpt0: <DPT Caching SCSI RAID Controller> rev 0x02 int a irq 10 on pci0.11.0
dpt0: DPT PM3334UW FW Rev. 07M0, 1 channel, 64 CCBs

Other arguments for trouble were:
 - defect disks
 - bad cableing
 - bad termination
