This is a practical observation, and does not eliminate the
theoretical possibility of the need to use more then two lists at
a time. Yes, the current shortcoming of available mailing list
software is indeed worked around by the careful crafting of the
mailing lists charters.

Well, if you say in advance "there's no way you could make this
work for me" about _anything_ I would not even try. Please, consider
changing your tone, those are not my "lazy-thinking crossposts"...

The habits appeared as a result of lack of certain features in
the mailing list software. But there really is not much to change.
The CC-ed reply will go directly to the addressee. But the mailing
list software will not send a copy to the same person. If that
person wishes to file the message -- she/he is free to do so manually
or automaticly (his/her mail processing scripts can gather plenty
of info from the headers). Currently used software has an option of
NOT e-mailing a message to its sender if the sender is subscribed
to the list. Taking this further is only natural, IMHO...
