Easiest way to trigger it: a) run virtualbox 5.2.34 with pulseaudio for sound on the VM (requires compiling it in) b) using a windows 7 VM, play anything that does multimedia [skippy audio, different problem most likely]. Let it run until audio stops working. After this, nothing else can access audio, either. This was happening with a 7/15/2019 kernel [forget which build number]. I recently updated to the latest 12-stable. So= und has been working all along since I built it last year, and no other notable problems observed.
