The short answer: none of your questions has a short answer. A recommendation: get Kirk McKusick's "Design and Implementation of 4.4BSD", it has a lot of the information you need, and a quick skim through the relevant chapters should be enough to give you an idea of the answers you need. I think that for most people, the ability to really answer even half of your questions would come after several university-level courses in computer science and OS design, and/or after several years' worth of work in the field.
