I'd highly recommend MailScanner (http://www.mailscanner.info) combined with SpamAssassin (http://www.spamassassin.org) and ClamAv (http://www.clamav.net/). The great thing about MailScanner is that it doesn't use milters, so you don't have to wait for a program to fire up and risk sending back temporary failure error codes. As long as your disks can keep up and you don't run out of queue space, it doesn't matter how long MailScanner takes to process messages.
