Open the callout device with O_EXCL.

The lock files are only for those programs that need to open callout
devices where they should use callin devices.  mgetty (or vgetty) are
examples of programs that need to do it, since they wish to watch
modem messages before the carrier is established.

Regular getty uses the dialin device, where the open() blocks until
the carrier is detected.
