NO_MAN controls whether we build/install the manual pages. Similarly the NO_GROFF option isn't directly related to NO_MAN. I suggest that to achieve what you want you use the NO_TOOLCHAIN option, only with installworld. It will work correctly, and not cause Groff to not be installed.
