For some time we have been having a problem with a FreeBSD 2.1.6 system,
the problem is that every once in a while (about every two days now) the
system hangs with a stream of messages on the console:

Assertion "cp" failed: file "../../pci/ncr.c", line 5568
sd0(ncr0:0:0): command failed (4 28) @f09d9400

Does anyone know what exactly this error means (is this a hardware
error on the disk, or the controller, or something else?), and what could
be done about this? We already have replaced the NCR controller without
result. Would upgrading to 2.1.7.1 help?
