Final followup.  With an Adaptec 2940, the system boots fine with a
CD in the drive.  It was some strange interaction with the NCR card
and the drive's response when there was a CD loaded at boot.

Mounting the CD once the system is up seems to be normal, and there weren't
any errors or delays booting with an empty drive.

It seemed to happen with all the CDs I tried.
