Daniel Ortmann <ortmann@sparc.isl.net> was closest to identifying the 
*real* problem.  I had a some typos in my configuration file:

options IPFIREALL
#options SAFETY
options NO_SCSI_SENSE

I couldn't find any docs on the "HCF" option :-(
