I finally cleaned up the support I added months ago for support of ISA cards
by the oltr driver.

This is for -STABLE but I will probably later update for -CURRENT as well.

The driver seems to be every bit as solid as before, but I welcome testing
and feedback.

I will probably continue to work on these a bit more to clean up a few other
things.
