Andy,
you can't have the same value for 'usrdeviceunit' on both entries
on the second entry change it to 1

...and about permanent both B channels:
in ppp.conf:
remove 'set autoload x y z' and modify 'link * set mode auto'
to 'link * set mode ddial'
