What do you think about adding --with-pam to the CONFIGURE_ARGS of the sudo port? After that it should be possible to just add a sudo entry to /etc/pam.conf and use any PAM supported authentication scheme with sudo. I'm successfully using sudo with pam_smb to authenticate against an NT domain this way.
