And yet another revision that works just fine...

(ncr0:4:0): "TOSHIBA CD-ROM XM-3501TA 2694" type 5 removable SCSI 2

I've never had any problems with this with any release of
FreeBSD. I used to have it attached to a 1542C controller which
worked fine too.
