a) What example config exactly? From the handbook? I did that.
b) My ppp.conf contains 
    set authname <myid>@bellatlantic.net
    set authkey  <mypasswd>
   is there something else I should do to enable PAP? As far as I know, PPP
   negotiates PAP automatically if these two parameters are present
c) I am using their cable. Again, as far as i can tell, this cable is no
   different from ordinary one - was able to replace their cable with older
   cable I have and DSL still worked OK under Windows 98
