Oh boy, that sounds very familiar. We have an IBM Ultrium 3583 (~ OEM Scalar 100) with two LTO drives and have had it both SAN attached (via a gateway), but currently attached to an Adaptec 29160. While we've experienced a plethora of (reproducible) error messages on both the Qlogic HBA and the current SCSI, with SCSI what seems to have helped was to set the card to 40Mhz/8-bit for the changer, and the two drives, and disable Int13. Obviously checking the cables and termination is the first option, but I assume that's been done

We're using SuSE Linux 8.1 with updated kernel but with (SuSE) standard Adaptec drivers, Rev 6.2.8 in this case. What SCSI card exactly are you using, what server is it?
