The device is accepting a command, disconnecting, and failing to reconnect
and complete the command within the timeout period.  The command that
failed was a read, so it could be that your tape drive is taking too
long to recover a faulty block so we timeout.  Do you get the same behavior
after cleaning the tape drive or using a new tape?
