Building mutt against the ncurses port was the cause. I pkg_delete'd mutt, ncurses port which also removed urxvt. Then started from scratch in terms of building mutt, this time against the base ncurses library, the re-installed urxvt and ncurses port as its depency and now all is back to normal. This is really for the archives and the mutt maintainer in case he wants to look into it but it's not a problem with the mutt port as such.
