This one works well for me, on 4.0-RELEASE:

mlx0: <Mylex version 4 RAID interface> mem 0xf5800000-0xf5ffffff irq 11 at
device 15.1 on pci0
mlx0: DAC960PTL1, 1 channel, firmware 4.07-0-29, 8MB RAM

I think this model is still available.
