The complete text of the Makefile is

MASTERDIR=      ${.CURDIR}/../gcc41/
PKGNAMESUFFIX=  -withfortran
WITH_FORTRAN=   yes

DESCR=  ${.CURDIR}/pkg-descr
PKGMESSAGE=     ${.CURDIR}/pkg-message

.include "${MASTERDIR}/Makefile"

You'll also want to do

cat > /usr/bin/f77
#! /bin/sh
/usr/local/bin/gfortran $@
