This means that the drive believes that tagged and untagged commands
were mixed, or two commands were sent to it using the same identifier.
Since you say you are using the FAILSAFE option, tags should be
disabled, so this is truely puzzling.  What kind of drive is this?
Have you checked with the vendor to see if updated firmware is
available for it?  Perhaps Stefan will have some ideas about this.
