Looks like the slice table got mulched too.

Are you sure you didn't have a double-disk failure, or that the 3ware has
finished rebuilding the volume? There's a lot of data missing for a single
disk failure in a raid5.  In particular, it looks like the whole first
shunk fo the volume is missing.
