It is Scotty, /usr/ports/net/scotty. Take a look at the scotty/tkined
homepage:

MRTG is nice too, you could monitor the SNMP device from Web page,
http://ee-staff.ethz.ch/~oetiker/webtools/mrtg/mrtg.html.
