remove superlu_mt option from cad/tochnog

tochnog hasn't maintained the superlu_mt interface (or much else) in a long
time.

tochnog currently has an optional dependency on superlu_mt 2.0. Unfortunate=
ly
even SuperLU MT 2.4 has changed the interface and the new interface is not
trivial to update. Furthermore some tochnog files are in a weird format wit=
hout
control returns for which I can't generate clean diffs.

I tried to build SuperLU MT 2.1, 2.2, and 2.3: all had problems or wanted to
build with OpenMP and ultimately I think it is just not worth it.

Let's just drop the option as it won't work when we bring in the modern
superlu_mt 3.1 version.
