There was an assertion a couple months back that for mps cards, the
firmware version must match the driver version, and that v14 was a
substantial improvement:

We're on 13 for both firmware and drivers on all our machines that
haven't had problems; a couple that are on firmware 09.00.00.00 with
driver 13.00.00.00 have had problems under heavy I/O (haven't been
able to get downtime to upgrade the firmware yet).
