You could use xvidtune in order to print out the desired settings for your monitor/adapter combination. I know that I did do a from scratch optimal resolution/frequency using a step by step linux howto included in the docs distributed with XFree86-3.3.x. Although if I remember correctly it did require that you know both the horizontal and vertical restraints of the monitor to achieve the accurate calculations.
