It looks like you're not running a rebuilt kernel... I suggest you try
recompiling your kernel again (do a config -r just to be safe).

If you still have problems, try a cd /dev && sh MAKEDEV all, but this
doesn't look to be permissions problems (which MAKEDEV all will fix).
