I'd try do "mt erase" and also replace tape by > another 4GB tape but it seems same way :( Hm, I'm usually quick to blame SCSI problems on termination and cabling first. Please check the termination is *absolutely* correct (termination on on devices at the end of the bus, termination power should come from the bus) and that your cable is of high quality.
