/usr/X11R6/lib/libXext.so
/usr/X11R6/lib/libXext.so.6
/usr/compat/linux/usr/X11R6/lib/libXext.so.6

/usr/ports/x11/libXext/distinfo
/usr/ports/x11/libXext/pkg-descr
/usr/ports/x11/libXext/pkg-plist

pkg_which on

/usr/X11R6/lib/libXext.a, /usr/X11R6/lib/libXext.so, /usr/X11R6/lib/libXext=
=2Eso.6

all give:

XFree86-libraries-4.3.0_6

Is this the same for you? Depending on your answer, portupgrade -f=20
XFree86-libraries might help.
