In this case, check_ping - from the standard check plugins [distinct from Netsaint] could be used to respond with a WARNING or CRITICAL state depending on RTA or packet loss ratio. Both Netsaint and plugins are in ports collection. In your case, acute episodic poor performance, you might be better served by intensive monitoring with MRTG or a continuous ping and a packet capture.
