CPU: EV56 (21164A) major=7 minor=2 extensions=0x1 OSF PAL rev: 0x1000800020117 real memory = 265904128 (259672K bytes) avail memory = 252829696 (246904K bytes) Replaced working controller (ncr0: ) with 53C895 (Tekram DC390-U2B) and new Cheetah drive. Booted from floppy and installed 4.1 via FTP without a problem. At reboot, there's no bootable drive available. SRM is the latest I could find [Digital AlphaPC 164 500MHz Console V5.5.1 Jul 14 1999]. Controller is and and it works in a PC - sees the drive. Searching the archives didn't turn up anything useful.
