The most common error is for the server to return an authoritative
NXDOMAIN response to an AAAA (or A6) query. The culprit seems to
be one particular DNS server (written in perl IIRC). The author of
the software was informed of the problem and a fixed version is
available. So I would ask first if they are running a perl-based
DNS server, and if the answer is "yes" then advise them to install
the latest version.
This was discussed within the last few weeks across a couple of
mailing lists. I don't recall which specific lists, but you could
try digging through a NANOG archive.
