Move up to 2.2.6 or 2.2-stable.  Configure a kernel with the pnp
device.  Also, I found I also had to modify
/sys/i386/isa/sio.c just slightly to make it recognize the ModemSURFR
56K internal as a modem, and I had to override the configuration
chosen by the BIOS by booting with the -c flag.
