Check out the MRTG home page. http://ee-staff.ethz.ch/~oetiker/webtools/mrtg/mrtg.html It works by querying devices using SNMP then turning that information into graphs using the GD library. If you need more than that then download it and read the source - it's writtern in perl and C. From memory it's fairly easy to setup - and I was doing in on a Windows NT box. Just read the doc.
