From what I understand, if using qmail with Maildir, then you can only use Courier-IMAP (in ports and also <http://www.courier-mta.org>), not Cyrus or UW IMAP (at least without patching the last two). Actually, I'm in the middle of trying that myself. Using qmail (with Maildir) and trying to get Courier-IMAP to work.
