Short story: After playing around with things I have discovered the problem is with the ctwm window manager. Long story: After seeing the above report that a successful start of firefox results in the same messages, I started looking at other things. One long standing issue I've had with ctwm is I have to do a restart (f.restart) to get virtual screens to work. So I did a restart of ctwm and saw the firefox window briefly flash on the screen.
