(Well, almost never (G&S: Pinafore)) Anything fails, but most mainframe components can be more easily engineered to provide fault-tolerance. My primary employer has several mainframes "custered" with Sysplex for about a half-dozen large hosts. There have been failures and they have been memorable, but very few.
