the firmware seems a bit old ....however I'm running a couple of these : dac960ptl firmware 4.08.0-33, bios 4.1, 32mb edo cache flashrom 512kb with 18gb disks and although it seems counter intuitive, try setting bios/head translation option of mylex card to 2gb geometry NOT >8gb.
