Annelise's book being missing is my fault - she sent me some copy, and an image to use for the site a long long time ago, but you've only just brought it out of the dusty vault in the pit of my brain. They are on a backup somewhere, which I will dig out in a week's time (I am going on holiday in about 8 hours time). I did mail Ted Mittelstaedt (sp?) about his book, but didn't receive a response, if I remember correctly. Sorry for dropping the ball on this one.
