By bisecting, I found the first "bad" commit for this bug is:

(it changed EFI_STAGING_SIZE from 32MB to 48MB).

It looks Windows Server 2012 R2 Hypervisor doesn't support UEFI VM boot with
the 48MB? Trying to dig into this. Any help is appreciated!
