Is the requested hostname (www.domain.com above) the same as the "ServerName" specified in your apache httpd.conf file? You may need to set "UseCanonicalName off" in httpd.conf . Read the relevant Apache docs (e.g., http://www.apache.org/docs/mod/core.html#usecanonicalname).
