Since I couldn't find the recent snapshot on ftp.freebsd.org, I decided
to try 5-STABLE kernel with mpt driver from 5.5-RELEASE first, and that
didn't have the problem (5-STABLE already has MFC'ed mpt driver, and it
has the I/O error problem). I did find a snapshot from September 5th,
and it still has the same problem. So I'm sure it's something in the new
driver, but at the moment I don't have a clue on how to investigate any
further.

FWIW, I tried to collect some information and put them here:
  http://les.ath.cx/FreeBSD/mpt/

I'll try to get some more debugging information the newer mpt driver later.

Best regards.
