OK, UK addresses take this format (I've put the US field names in
``()'' to show where the problems are):

   (address)  151 Main Street
	      Village
 (town/city)  Town
     (state)  County
       (zip)  AB12 3CD

As you can see, we need 2 "address" fields and the "state" and
"zip" fields need to be longer. 

The telephone number format is:

01234 567890

or in major cities:

0123 456 7890

That would be up to the individual I guess. Personally I would be
prepared to do so although when the registrations start arriving
at the rate of 100 per day :-) the postage cost will get a bit
prohibitive :-(.
