That means formatting has been incomplete.  You need to find out
why.

Try the FreeBSD fixit floppy.  I think scsiformat is missing on it,
but scsi(8) itself should be there.  Try:

	scsi -s 3600 -f /dev/rsd0.ctl -c "4 0 0 0 0 0"

(and get a cup of coffee afterwards).

At least, if FreeBSD fails to format it, you'll get a useful SCSI
error message...  I think if formatting repeatedly fails, it's really
your drive that went south now, for whatever reason.
