You have a set of floppies for each release. The tree for 4.0-Release starts at ftp://ftp.FreeBSD.org/pub/FreeBSD/releases/i386/4.0-RELEASE/ You will find a "floppies" directory with kern.flp and mfsroot.flp in it. You have to write them on to a freshly formated floppy with fdimage from ftp://ftp.FreeBSD.org/pub/FreeBSD/tools/. The format has to be flawless, i.e., no bad sectors. You format so that it is right and consistent (aligned) with your floppy drive.
