from man 1 cvsup:

  If the supfile includes the command base=3D/foo the refuse files would be=
:

           /foo/sup/refuse
           /foo/sup/src-all/refuse
           /foo/sup/src-all/refuse.cvs:RELENG_3

In effect, yes, with base=3D/var/db your refuse file should be /var/db/sup/=
refuse.
